Welcome to the on-line pre-application page
for the 2005 SC Zone Team.
It is important that you read the DIRECTIONS. Accuracy is very important.
DIRECTIONS:
1. Read ALL the directions below before applying.
2. Read the Selection Criteria before applying.
3. Every on-line field must be completed. Please follow indicated formats for phone numbers, USA ID and birthdays.
4. Before you begin, know your USA ID number. Your USA ID Number is available on-line. Go to Connecticut Fast and select Swimmer- Best Times. The USA Swimming ID number is the 6 digits of your birthday i.e. 070491, followed by the first 3 letters of your legal first name, your middle initial and the first 4 letters of your last name. Susan V. Fast (nickname Suzie)= 070491SUSVFAST. If your legal first name or last name is less than 4 characters, use an *. If you have no middle name use an *. Apostrophes and hyphens are ignored.
5. If you would like to change your submittal or made an error, please email office@ctswim.org. Do not send another submittal.
6. Confirmation of receipt will be posted on ctswim.org
All pre-applications for 14/U athletes, including adapted, must be received by February 26, 2005. This deadline is firm and there are no exceptions. The CSI server (Spellnet) will time and date stamp each submittal. Submittals must be stamped by 11:59 PM February 26, 2005.
